A Federally Qualified Health Center (FQHC) can only establish in health care professional shortage areas. FQHC's must provide services to anyone regardless of their ability to pay. A sliding fee is provided to those who qualify. Throughout the country, FQHC's may differ in the services they provide, but the mission is always to address the health care needs of the uninsured.
Call the clinic nearest you to schedule an appointment. Staff will inform you of the necessary paperwork you need to bring to your appointment. Some of the necessary forms you need to complete can be found on this website under ‘Patient Tools.’
If applying for the sliding fee, you will need to bring 1040 and W-2 forms; proof of income such as paycheck stub or unemployment letter. If you have no income, you will need to bring a denial letter from the Medicaid office. If you have Medicaid, Medicare or private insurance, please bring a copy of your insurance card.
All medical providers accept Medicaid and Medicare. All dentists accept Medicaid. Insurance coverage varies by provider. Please call the clinic of your choice to find out which of our providers is a provider for your insurance.

No, our provider staff are full time physicians, nurse practitioners, dentists, dental hygienists, nurses, etc. Each clinic has a physician on staff, as well as adequate support staff.
Yes, in the interest of your safety and the safety of our staff, we may close or adjust operating hours during periods of inclement weather. Every effort will be made to contact everyone who has a scheduled appointment; however you should call the main number of the clinic in which you receive care for the latest information. You can also tune to the following radio stations for the latest information on clinic closings - KZIM 960 AM, KGMO 100.7 FM, 1550 AM, KMHM 104.1 FM.

Cross Trails Medical Center accepts cash, check, money order or MasterCard/Visa/Discover. Computerized statements are sent monthly. Standard collection procedures will be implemented for all past due accounts.
